My Mommy and I Mother’s Day Survey

This Mommy and I Mother’s Day Survey is very much a last-minute Mother’s Day printable that I had to kinda sneak in as the second post of the day {my printable I Love My Mom! Emergent Reader was the first one} to actually give you a chance to use it this year.

On the upside, there’s no preparation needed, you can have it filled in within minutes, and it’s a super cute keepsake. Plus if you didn’t see it in time for Mother’s Day, it’s also great for mom’s birthday, the first or last day of school or really any day of the year when you want to preserve some memories with kids of all ages.

The reason why I left this for the last minute? I really don’t like to do much if anything for Mother’s Day, because, well, I’m the mom. Do you feel similarly?

I’m making an exception for this survey because I really don’t have any input on what E answers, I’m just recording her words. Also, I just think it’s so cute, and I know I’ll love looking back on it in years to come. In fact, I hope to end up with a whole series both from her and N to make me all nostalgic in my old age 🙂

What’s in the Printable PDF File?

This is a one-page, no-prep printable questionnaire that makes a perfect mother’s day gift.

Print it, ask your child the questions and fill in the blanks. This way, you can work with children as young as preschool age. If your kids are older and able to, let them fill in the blanks themselves.

Please note that there will be a small white margin when you print. This is unavoidable due to how printers work, but you can easily trim off the white area with a paper cutter or scissors.

If you choose to laminate, I’d do it after the blanks have been filled in. Alternatively, the mom’s day survey also looks great in a frame.

The questions have a slight twist on other Mother’s Day surveys I’ve seen: They focus on both the child and the mom. For example, one question is “What’s your favorite activity to do with your Mommy?”, and another is “What’s your Mommy’s favorite activity to do with you?”.
